Thank you for agreeing to participate in the National AT Aging & Dementia Decision Tree/Algorithm Workshop, hosted by Tools for Life | Georgia Tech, in partnership with the Georgia Department of Aging Services (DAS) and CREATE. We are looking forward to working with you!

Goal:

  • The goal of this Workshop is to smart-source our awesome community of AT professionals to develop Decision Trees/Algorithms to expand knowledge of AT, AT strategies, resources and to identify "red flags" to ensure appropriate referrals. Ultimately, we want to assist everyone in finding appropriate Assistive Technology strategies and solutions for individuals who are aging and individuals with dementia.

The Details When: January 17th 11-4:30 January 18th 9:30-4:00

Where: Royal Caribe Hotel & Conference in Orlando, Florida

Who: You, your AT Act Program team who provide direct service, other AT Professionals and subject matter experts (SMEs) from across the country in a variety of fields to help develop the AT & Aging Decision Tool (Algorithms) and AT & Dementia Decision Tool (Algorithms).

We have researched several existing assessment tools and checklists that are currently available. If you have access to any additional assessments, checklists, flowcharts and/or decision-making tools that would be helpful for us to review, please share if you can. We will be adding these to our reference guide of all these resources and will make those available prior to the meeting for participants to review.

We are excited about this opportunity to create innovative solutions with You that help so many folks and have far reaching positive outcomes!

If you are interested/available able to join us in this important Workshop, please, let me and Liz Persaud know by November 30th.

Looking forward to collaborating with you on these important projects. Please, let me know if you have any questions.

Thank you for considering this request to share your knowledge and wisdom!
